Telephone Line

For ADSL connections, the name of the owner or contracting party of the phone line is required as a double-check that the service is being installed on the correct phone number. (Note: the name on the telephone bill means nothing--employers, for example, often arrange phone service in their name but have the employee's name put on the bill.)

Please answer the following questions about the phone line.

1. Did you personally contact NTT and make the arrangements yourself to have telephone service connected?
Yes, I arranged the connection myself.
No, someone else arranged the connection

If you did not personally arrange the telephone connection yourself, please provide the name of the person or organization (employer, landlord, etc.) that arranged or is providing the phone service. This will help us verify that installation is done on the correct number. If you do not know who arranged it, enter a comment about what you know about the phone line.

First (Personal) Name:

Current Type of Phone Service

In some cases, lines must be converted from ISDN or cleared of current service before ADSL can be installed. Please answer the following questions to see if that's the case on your line.

graphic showing modem-like device between phone and wall

1. Is there a device between your phone and the wall jack similar to the above?

If your phone has a direct wire from the telephone to the wall jack and does NOT have a device similar to the above, click here:
No device
If you selected "No device" here, skip both No. 2. ("What markings") and No. 3. (Other) and go to the next section.

2. What markings are on the device?

If there is a device between the phone and the wall, look at the markings on it.

If it says, "ISDN," "INS-64," "V-30," "ATERM," or "T/A," please select "a," "b," or "c" in the following list. If none of the above words appear on the device, skip to No. 3, "Other."

Conversion depends on ownership of the line. If any of the terms just above appear on the device, click one of these:
a. Regular or "owned." Regular means phone line was purchased.
b. Lite or "rental." Lite means the line was not purchased.
c. Don't know if line is owned or not.

3. Other

If there is a device between the phone and the wall and it is NOT marked with "ISDN," "INS-64," "V-30," "ATERM," or "T/A" then click here:
